ご予約について

上記の予約フォームにご希望の日時を第2希望まで入力してご送信ください。

尚、尚、初回は、これまでの経緯をお聞きしたり、主訴に関係する検査やチェックなどを行いますので初見料（問診・検査料3,300円）が含まれています。

当日の施術がスムースでより良くなるためにフォーム記入欄も多くなっておりますがご協力くださいませ。

2営業日以内に返信いたします。返信メールが届かない場合は、サーバーで止まっていたりPCの不具合なども考えられます。お手数ですが、一度お電話にてお問い合わせくださいませ。
